VIII. Keeping Track of Contracts & Completed Interviews

You are responsible for scheduling all of your interviews and follow-up interviews over a [sixteen month] period.  You will start with using a “telephone contact record” and a “field contact record” for each respondent to keep track of your contacts and completed interviews.  By the time of the first wave of interviews, you will keep track on an electronic database.  Beginning the second week, a copy of both forms, or the electronic submission, must be turned into the field coordinator every week.  A special box for collection of these forms will be kept in the locked file cabinet where the completed questionnaires are stored.  The [site] coordinator will review the forms and completed questionnaires at the beginning of each week.  (Forms for both the telephone contact records and the field contact records are in the Appendix).

Each respondent will have one Telephone Contact Record.  At each follow-up respondents will have another Telephone Contact Sheet.  The field contact record will list all of the respondents being interviewed on one sheet.  There is a separate field contact record for each follow-up interview with each respondent.
